Good questions. We are in the process of reorganizing the website, so these forms should probably be made more prominent. If you go to the main page of the website, which you can do by clicking on the banner at the top of the forum, look in the upper right, under where it says "Top Menu". Click on the Org. Chart.
At the bottom of the Org. Chart page there are links to various documents, one of which is the County Documentation kit. That contains the forms to recognize a county organization as well as a board of directors representative for the county.
At least two LPPA members in good standing must sign to have the organization recognized, and it needs at least a chair and secretary for officers. Twelve members in good standing need to sign to have a board representative.
The signed forms can be faxed to the 1-800-R-RIGHTS number, or if they can be made into a .pdf or similar electronic document, emailed to
secretary@lppa.org. A hard copy should also be mailed to the LPPA, 3915 Union Deposit Road #223, Harrisburg, PA 17109. That same should be done with any county constitution, bylaws, and/or platform adopted. All county affiliate organization must be compliant with the LPPA bylaws, which can also be downloaded from that section of the website.
I can email you a sample constitution and bylaws that can be modified if you like. Let me know if you have other questions.
