PRINTING INSTRUCTIONS

1. The nomination paper form must be printed as duplex (two-sided, single page, front and back) form on 8 ½" x 14" (legal size) paper. This requirement cannot be satisfied by printing each side of the duplex nomination paper form on a separate sheet of 8 ½" x 14" paper and affixing the two sheets together.
   
2. The nomination paper form must be printed on plain white 8 ½" x 14" (Legal-Size) paper.
   
3. Please read all instructions provided with the nomination paper forms prior to circulating them.

FILLING OUT THE FORM

You can always print the form as is, in accordance with the printinig instructions above, however this pdf file also has fields that can be edited as follows. 

1. The county field can be edited  Feel free to type in a county before printing.
   
2. If you are a candidate for U.S. Senate, Congress or the State Assembly or will be collecting signatures in one of our candidates districts, you can enter candidate information on the form prior to printing.  This information can be hand written on the form as well.  Enter the office title for the candidate i.e. Representative in Congress, Senator in the General Assembly or Representative in the General Assembly.  Then, enter the appropriate District number, name, place of residence and occupation.  
   
3. If you are using adobe reader, you will not be able to save any data you enter into the form.  You will only be able to print the form with your data, so it may be best to keep a clean copy in case you need more copies.

1. Signers must be registered voters.
2. They can be registered with any party or as an independent.
